Here’s how you can get started:
Begin by filling out a short, secure form. This form provides us with basic information about your case, which we will review to determine if our services are the right fit for you.
Step 1: Complete the New Patient Request Form

Step 2: Sign Legal Forms
After confirming your eligibility, we will send you the necessary legal forms, including the Genetic Counseling Consent and Financial Responsibility Agreement. These documents must be signed and returned before we can move forward.
Step 3: Choose Package & Payment
Next, you will select the service package that best fits your needs. Once you’ve chosen your package, you will proceed to make the payment securely online.
Step 4: Schedule Your Appointment
With your forms signed and payment complete, you will then schedule your first genetic counseling session. This appointment will be conducted via Google Meet, providing convenience and flexibility.
Step 5: Complete Medical & Family Health History Questionnaires
After scheduling your appointment, you will receive an email with the link to complete your medical history intake questionnaire in addition to your family health history via CancerIQ. These will need to be completed at least 24 hours before your scheduled appointment.
Submit Records & Previous Results

Depending on your health history, we may request specific medical records. This helps ensure a comprehensive review and more personalized recommendations.
